What you'll do

• Provide personal care and assistance with daily activities according to individualized care plans
• assist with meals, medication, hygiene, and mobility as needed
• plan and support activities and outings
• document activities, progress, incidents, and care
• follow safety and emergency procedures
• monitor health and report concerns
• implement behavior support strategies and therapeutic interventions as outlined
• communicate with residents, families, coworkers, and external partners.

Why this role
