What you'll do

• Complete patient intake and orientation
• assess medical, psychosocial, and psychiatric treatment needs
• monitor attendance and treatment progress
• advocate for services and obtain resources
• maintain case notes and records
• participate in weekly interdisciplinary meetings
• communicate progress and barriers to the therapeutic team and family
• prepare discharge plans and coordinate post-discharge requirements
• compile treatment reports and statistics
• support staff education and process improvement.
