Case Manager (CSS)

What you'll do

• Provide direct case management services to adults, usually weekly
• help clients identify and secure community resources
• coordinate referrals and services among community agencies, employment contacts, and the organization
• develop and implement individualized plans with clients and other involved parties
• maintain complete service records and submit required documentation
• build knowledge of community resources
• maintain professional relationships
• attend workshops and seminars. Regular travel within the service area is required. Requires an associate’s degree plus two years of health and human services experience or four years of such experience
• case management training certificate must be provided within six months of hire.
