Registered Nurse — Home Health

Job overview

The Registered Nurse — Home Health role is an onsite position in Los Angeles, CA, with part-time employment. Compensation is $50–$60 per hour. The nurse reports to the Director of Patient Care Services, Director of Nursing, or Nursing Supervisor and provides home care services to improve patients’ quality of life. The nurse assesses patients, develops and updates individualized plans of care, administers ordered medications and treatments, and coordinates with physicians and other care providers. The role also supervises LVNs and CHHAs, educates patients and families, and maintains timely clinical documentation.

What you'll do

• Complete initial and ongoing patient assessments
• develop and revise individualized plans of care with measurable goals
• determine and document nursing diagnoses and interventions
• administer ordered medications and treatments
• report changes in patient condition
• supervise LVNs and CHHAs
• educate patients and families
• coordinate care with providers and physicians
• submit clinical notes
• participate in QAPI initiatives and in-services
• follow agency policies and HIPAA requirements.
