The Home Health Registered Nurse (RN), Napa Area role is an onsite field position serving Vallejo, Benicia, Napa, American Canyon, and Yountville, CA. Compensation is $90,000–$160,000 USD per year for the full-time schedule. Paid training is onsite in San Ramon, CA, for five days, Monday–Friday, 9:00 a.m.–5:30 p.m. The role supports CVHCare’s home health clinical services. The nurse monitors patients’ overall health, partners with primary care physicians on individualized care plans, and manages cases independently in patients’ homes. The role contributes to quality home-based care through patient education and specialized wound, post-surgical, and ostomy services.

What you'll do

• Monitor patients’ overall health
• partner with primary care physicians to develop individualized plans of care
• manage patient cases independently
• provide customized care and education about disease management
• deliver simple to complex wound therapy, post-surgical care management, and ostomy care management
• coordinate patient visits and schedules.

Why this role

Full-time schedule is 24 RVE per week; part-time option is 18 RVE per week. Must be available to work up to two weekends per month. Requires an active California RN license, at least one year of RN experience, reliable transportation, and current CPR or BLS card.
