What you'll do

• Deliver professional nursing services in clients’ homes and provide case management for primary patients
• conduct initial assessments and monitor progress and clinical outcomes
• develop and monitor plans of care with patients or representatives
• communicate with patients, families, physicians, other disciplines, clinical managers, and office staff
• participate in interdisciplinary conferences
• complete OASIS, evaluations, orders, and care coordination documentation
• coordinate with physicians and discharge planners
• promote health education
• and liaise with community health resources including DME, oxygen, and infusion providers.
