What you'll do

• Facilitate intake, screening, and assessment in office, by phone, in hospitals, at clients’ homes, or in other healthcare settings
• integrate person-centered planning
• complete assessments and care plans
• coordinate services and liaise with treatment teams
• educate clients about medication side effects and lab results
• address financial and psychosocial barriers
• complete encounter notes within 24 hours and maintain electronic health records
• maintain confidentiality, licensure, and credentialing requirements
• participate in onboarding and professional development.
