Nurse Case Manager (Licensed Practical Nurse) is a full-time hybrid role requiring three onsite days per week at the Flagship location in Brooklyn, NY. In-office hours are 10:00 a.m.–6:00 p.m.; compensation is $38.29–$39 per hour. The role supports Little Flower’s Enhanced Family Foster Care program and coordinates healthcare for children and youth in foster care. The nurse coordinates medical and mental health care, develops and monitors individualized care plans, and advocates for children and youth from birth through age 21. The role works with foster parents, families, casework staff, schools, and providers to support health, safety, and ongoing care.

What you'll do

• Assess, plan, implement, and monitor foster children’s healthcare
• coordinate care and appointments
• conduct developmental, intake, and other assessments
• participate in scheduled onsite well-child clinics
• provide health education and training to children, youth, and foster parents
• support chronic-condition management and emotional or psychiatric needs
• establish treatment goals with the multidisciplinary team
• follow up after appointments and urgent care
• maintain medical documentation
• assist with home and community provider visits as needed
• communicate with treatment teams
• follow regulatory requirements
• maintain supplies and practice infection control.
