What you'll do

• Support local WIC and immunization clinics
• help people navigate healthcare systems and insurance benefits
• provide culturally sensitive health education, basic screenings, and healthy-lifestyle coaching
• connect clients with community resources and social services
• serve as a liaison and advocate between clients and providers
• document interactions and community needs
• support coordinated preventive care and community partnerships.
