What you'll do

• Identify potential skilled-care candidates and confirm eligibility
• facilitate notices and care-status discussions
• obtain physician orders and certifications
• maintain the skilled-care census log
• coordinate assessments and complete the RAI-MDS
• supervise and coordinate the Activities Program
• maintain CMS-compliant care plans
• support discharge planning
• document in the electronic medical record
• participate in quality improvement
• communicate with patients, families, physicians, and staff
• provide RUG and billing information
• assist Utilization Review Nurses.

Why this role
