Hospice Field Registered Nurse (RN) — PRN

The Hospice Field Registered Nurse (RN) — PRN is an onsite field role based in Overland Park, KS; compensation is not specified. Choice Health At Home provides hospice care to patients in their homes or other care settings, supporting comfort and dignity for patients and their families. The nurse assesses patient needs, develops and documents care plans, coordinates with an interdisciplinary team, and educates patients and families. The role also directs hospice aides and LPN/LVNs, monitors responses to care, and provides on-call service as assigned.

What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine hospice admission and recertification eligibility
• develop, implement, and document care plans
• educate patients and families
• assess caregiver ability to meet patient needs
• provide clinical direction to hospice aides and LPN/LVNs
• prepare clinical and progress notes
• communicate changes to physicians and other personnel
• perform on-call responsibilities as assigned.
