The Bilingual Community Doula – Doula Navigator role is based in Tennessee, with the listing location in Nashville, TN; patient contact may take place by phone, text, video, or in person according to patient preference. The role uses trauma-informed, evidence-based communication and care-management tools to support patient learning, follow-through, and maternal health outcomes.

What you'll do

• Conduct outreach, check-ins, and comprehensive assessments
• assist with appointments, office visits, and social-service applications
• coordinate care services and referrals with health plans, providers, and multidisciplinary teams
• use motivational interviewing, trauma-informed care, and harm reduction
• document services in care-management portals and manage caseload follow-up.

Why this role
