The Habilitation Coordinator, Day Hab role is onsite in Rochester, NY, at CP Rochester’s Winton Road facility. The stated pay is $19.50–$21.50 per hour. The schedule is full-time, 35 hours per week, typically Monday–Friday between 8:00 a.m. and 4:30 p.m., with occasional early-evening meetings. The role supports people with disabilities in pursuing productive and rewarding lives. The coordinator develops and maintains person-centered plans and records, supports individual choice, and helps program staff implement individualized action plans. The role monitors outcomes, maintains regulatory documentation, and collaborates with individuals, families, and the supervisory team.

What you'll do

• Develop and maintain individual charts and program documentation
• build partnerships with individuals, families, and the community
• advocate for meaningful choices
• attend Life Plan reviews
• update Staff Action Plans and Individual Plans of Protection
• train program staff on documentation and plans
• evaluate plans through reports and assessments
• maintain regulatory records
• provide direct care when needed.
