What you'll do

• Assess, evaluate, and implement individual health and behavioral plans for chronic illness, including Type II diabetes
• document in the electronic medical record
• facilitate group classes that meet American Diabetes Association requirements
• provide weight management therapy for adult patients
• provide community health education and identify healthy resources
• participate in continuous quality improvement
• travel to Kintegra sites and community events.
