The IMPACT Case Manager role is onsite at the 5th Street location of Community Support Services in Sioux Falls, SD. Compensation is not specified. Southeastern Directions for Life’s mission is to empower people to discover their directions for life in a changing world; this role serves adults with severe mental illness in an Assertive Community Treatment program. The case manager provides direct service, assessment, and service coordination to support clients through recovery. The role also supports medication management, life-skills development, and recovery through trusting relationships and coordinated team care. The schedule consists of 10-hour shifts, Wednesday–Saturday or Sunday–Wednesday, with participation in a rotating on-call system.

What you'll do

• Provide direct service, assessment, and service coordination to adults with severe mental illness
• assist with medication management after training
• support life-skills development and recovery
• establish supportive, trusting relationships
• participate in the rotating on-call system
• work 10-hour shifts on the Wednesday–Saturday or Sunday–Wednesday schedule.
