Maternal and Infant Health Community Health Worker

What you'll do

• Build supportive relationships with patients and families
• assist with approved screenings
• share resource information and make referrals
• help with applications and appointment engagement when appropriate
• follow up on referrals and identified needs
• participate in outreach and community activities
• collaborate with the interdisciplinary care team
• document services and grant activities
• protect patient information
• and escalate urgent or high-risk concerns according to policy.
