What you'll do

• Conduct initial contact and assessment visits
• evaluate hospice eligibility and coordinate with hospice physicians
• obtain informed consent and explain patient rights and responsibilities
• develop the initial interdisciplinary plan of care and complete documentation
• instruct caregivers
• address immediate concerns and provide symptom-management treatment onsite
• coordinate medications, supplies, and equipment
• participate in interdisciplinary group conferences and manage care between admission and transition to a nurse case manager.
