The Certified Nursing Assistant (CNA) – Day Center role is an onsite position in Riverside, RI. Compensation is not specified. The role supports participants of the PACE Organization of Rhode Island under the supervision of the Day Center Director. The CNA provides personal care and services according to individual care plans, supports participants’ safety and daily activities, and documents care to help meet their needs across the Day Center and assigned community settings.

What you'll do

• Provide personal care including bathing, toileting, grooming, oral care, and eating assistance
• check vital signs and monitor weight as directed
• follow participant care plans and nursing instructions
• support behavioral supervision and restorative activities
• respond to personal alarms
• maintain care logs and electronic health records
• assist with meals, laundry, recreation, and personal care supplies
• maintain clean and safe areas
• provide in-home care and escort participants to appointments as assigned
• attend care planning meetings as requested. Rotating weekends and/or holidays may be assigned.
