What you'll do

• Counsel residents and families on illness-related changes, grief, loss, placement, and depression
• develop resident-centered care plans with the interdisciplinary team
• assist with advance directive planning
• advocate for residents
• attend facility meetings
• protect residents from abuse, neglect, and misappropriation
• complete required documentation
• support census and prospective resident processes
• educate residents and families about financial and insurance matters
• assist with employee orientation on resident rights
• and supervise the Social Services team.
