The Registered Nurse (RN) – Private Duty Home Health, PRN role is onsite in Palm Harbor, FL, serving the Pinellas/Pasco County area and traveling to clients' homes. Pay is $45 per visit and $45 per hour. Mind & Mobility helps seniors live independently and with dignity through in-home care, personalized therapy, and brain health programs. The nurse assesses clients, develops individualized care plans, and evaluates caregiver skills through supervisory visits. Patient and family education, timely documentation, and communication with the care team support safe, personalized care in the home setting.

What you'll do

• Assess patients' health status and needs in the home
• develop and implement care plans with patients, families, and caregivers
• educate patients and families on disease management, health maintenance, and safety
• communicate progress and changes in condition
• maintain accurate and timely documentation
• promote a safe and supportive home environment
• evaluate caregiver skills and conduct supervisory visits to guide care delivery and ensure care-plan compliance.
