The nurse organizes and directs hospice care during assigned after-hours coverage, assessing patient and family needs and responding to changes in condition. The role coordinates care with physicians, caregivers, and the interdisciplinary group, providing nursing interventions and continuity of care during nights and weekends. The schedule is seven days on and seven days off, with Monday–Thursday coverage from 5:00 p.m. to 8:30 a.m. and Friday 5:00 p.m. through Monday 8:30 a.m.
What you'll do
- Complete initial and ongoing comprehensive assessments of patients and families
- respond to triage calls and make visits for changing conditions
- complete weekend admissions
- provide nursing care and develop or revise plans of care
- administer prescribed medications and treatments
- counsel and instruct patients and families
- remain available for patient-care calls and make visits as needed
- document clinical notes and changes
- communicate with physicians and community health contacts
- coordinate with the hospice interdisciplinary group
- supervise and delegate to ancillary personnel when required.
