Case Manager – Shelter

Job overview

The Case Manager – Shelter role is onsite in Rockville, MD. Compensation is $55,000–$65,000 per year. Interfaith Works’ Homeless Services department supports people experiencing homelessness in pursuing stable housing. The case manager provides housing-focused support to a caseload of approximately 25 participants, helping them identify goals, address barriers, and connect with housing and community resources. The role contributes to stabilization through participant-centered planning, coordinated services, crisis response, and timely documentation.

What you'll do

• Maintain a caseload of approximately 25 participants and meet with them at least weekly
• assess needs and strengths
• develop and update service and housing plans
• support housing applications and navigation
• connect participants with community resources
• coordinate with providers and agencies
• participate in Coordinated Entry and interdisciplinary meetings
• maintain accurate HMIS records
• provide crisis intervention and follow safety protocols
• work at least one evening per week and facilitate or co-facilitate a community meeting every other week.
