Registered Nurse (RN) Case Manager

What you'll do

• Assess the impact of terminal disease on patients’ physical, functional, psychosocial, and environmental needs
• implement and recommend revisions to individualized plans of care
• educate patients and families about disease processes and end-of-life care
• document clinical progress
• coordinate services with the interdisciplinary team
• communicate changes to physicians and other personnel
• assess caregiver capacity
• determine service scope and frequency
• perform clinical tasks and assigned on-call responsibilities.
