The role also requires documenting each shift and reporting changes or concerns to support consistent care.

What you'll do

• Follow the client’s care plan and agency policies
• assist with personal care and daily routines, including bathing, dressing, grooming, and toileting as needed
• provide mobility and transfer support, companionship, and supervision
• prepare meals and snacks
• provide medication reminders
• perform care-related light housekeeping
• assist with authorized errands or transportation
• maintain a safe and comfortable home environment
• observe and report changes or concerns
• complete shift documentation.
