What you'll do
- Ensure current care plans and MDS for residents
- oversee scheduling, completion, and accuracy of MDS item sets
- coordinate written resident care plans and multidisciplinary approaches
- chair care-plan conferences and relay concerns
- contact residents and families about conference participation
- support RAI education and orientation
- monitor medical records for consistency and content
- maintain diagnosis codes
- report IT concerns
- participate in the facility QAPI program.
