What you'll do

• Provide skilled nursing services under the plan of care
• conduct initial and ongoing evaluations
• initiate and revise care plans
• document visits and care conferences
• coordinate services and communicate patient changes
• counsel patients and families
• process orders and notify physicians
• complete certification, recertification, discharge, and OASIS documentation
• supervise and teach nursing personnel
• participate in assigned reviews and in-service programs.
