What you'll do

• Assess patient needs and discharge barriers
• develop and update care plans with the interdisciplinary team
• initiate discharge planning at admission and coordinate post-acute services
• conduct utilization reviews and assess medical necessity and level of care
• support denial prevention, management, and appeals
• monitor payer and regulatory requirements
• complete documentation
• participate in performance improvement
• monitor outcomes and transition metrics
• advocate for patients and families.
