MDS Coordinator

What you'll do

• Schedule, set up, and complete MDS assessments
• coordinate new-admission authorizations and updates with families, insurers, and case managers
• code new admissions and update long-term-care records using ICD-10
• participate in resident care-plan implementation and review
• attend care conferences
• educate the interdisciplinary team on MDS, assessments, skilled-care requirements, and documentation
• run utilization review meetings and validate PDPM scores and rates
• participate in quality measures and QAPI
• conduct chart reviews and audits
• assist during surveys.
