Registered Nurse Case Manager and Recertification Nurse, Home Health

Registered Nurse Case Manager and Recertification Nurse is a field-based role serving Northwest Ohio, with part-time or PRN options and weekday daytime availability, generally 8 a.m.–5 p.m. Pay is $36–$40 per hour plus mileage. The nurse-owned Home Care Network supports home health clients across the region. The role coordinates and reassesses home health services, confirms ongoing medical necessity, and provides nursing care to support patient outcomes. It also leads field staff, communicates patient status across the care team, and maintains timely, compliant documentation. Travel within the selected service radius is required; overnight travel and on-call duties are not required.

What you'll do

• Manage cases, starts of care, admissions, recertifications, transitional care, resumptions of care, clinical reviews, and assessments
• provide occasional backup skilled nursing visits
• conduct client assessments and initiate appropriate actions
• supervise RN, LPN, home health aide, and personal care aide staff
• document and communicate patient status and changes
• coordinate care with physicians, agency staff, administrators, caregivers, clinicians, clients, and families
• complete compliant documentation.

Why this role

Requires an active Ohio RN license, at least one year of RN experience and home health experience, CPR and First Aid certification, OASIS and non-OASIS assessment experience, and nursing supervisory experience. Weekdays Monday–Friday, primarily 8 a.m.–5 p.m.; occasional evening availability. No weekends required; weekend services are optional.
