Job overview

The Residential Service Assistant – Group Home role is onsite in Sussex County, DE; compensation is not specified. Horizon House provides community-based services to adults with developmental disabilities, behavioral health needs, substance use challenges, and homelessness. The assistant supports residents’ daily living skills, community participation, and independence. The role documents services and coordinates with treatment team members, families, and other supports to contribute to consistent program service delivery.

What you'll do

• Reinforce skill development in food management, personal hygiene, relapse prevention, medication management, money management, and planned community activities
• complete progress notes and service plans
• communicate with treatment team members, instructors, consultants, medical professionals, and family members
• support program service delivery
• provide crisis intervention in emergencies.

Why this role

The schedule is 40 hours per week in 8-hour shifts: 8:00 a.m.–4:00 p.m., 4:00 p.m.–12:00 a.m., or 12:00 a.m.–8:00 a.m. Preferred availability includes every other weekend on a rotating schedule.
