What you'll do

  • Oversee MDS processes across assigned skilled nursing facilities
  • ensure timely and accurate assessments
  • monitor PDPM classification and reimbursement integrity
  • audit assessments, care plans, and supporting documentation
  • identify risks and coordinate corrective action plans
  • train and mentor MDS coordinators and interdisciplinary staff
  • provide education on RAI updates, PDPM changes, and regulations
  • monitor quality measures, Five-Star ratings, and survey outcomes
  • support documentation and care planning improvements
  • prepare compliance, reimbursement, and performance reports
  • assist with survey preparation and follow-up.
