Registered Nurse (RN) MDS Coordinator

The Registered Nurse (RN) MDS Coordinator role is onsite in Cleveland, OH, with compensation of $40 per hour. The position supports the skilled nursing facility team at Slovene Home for the Aged. The role exists to coordinate accurate, timely resident assessments and individualized care planning while supporting quality outcomes, regulatory compliance, and appropriate reimbursement. It works with nursing leadership and the interdisciplinary team to maintain complete documentation and prepare for surveys.

What you'll do

• Coordinate and complete MDS assessments and the Resident Assessment Instrument (RAI) process within required time frames
• develop and update resident care plans with the interdisciplinary team
• monitor payer requirements, quality measures, and facility indicators
• review records for documentation accuracy
• support survey readiness
• and educate staff on MDS documentation and regulatory updates.

Why this role

Monday through Friday schedule with weekend availability as needed. Current unrestricted Ohio RN license or a license valid for practice in Ohio is required; RAC-CT certification or other relevant MDS education is a plus.
