What you'll do

• Conduct weekly in-home visits to assess child safety, risk factors, and family needs
• use a solution-focused approach
• refer families to parenting education, in-home assistance, medical and behavioral health care, and other supports
• reinforce family strengths and protective factors
• maintain accurate documentation
• collaborate with internal teams and community partners
• work flexible hours, including evenings and weekends as needed.
