Healthcare Coordinator

What you'll do

• Advocate for participants during medical appointments, procedures, and healthcare interactions
• transport participants to and from local appointments
• liaise with healthcare providers and internal nursing staff
• build relationships with community providers
• advocate for participant needs during routine and emergency situations
• maintain timely health documentation
• support mobility needs
• follow infection control and safety procedures
• collaborate with nursing staff and team members.
