What you'll do

• Support individuals’ emotional, physical, and personal well-being
• promote rights, inclusion, and community participation
• implement habilitation plans and document progress
• assist with daily living as needed
• report suspected abuse or neglect
• communicate with families and service coordinators
• attend required meetings and training
• and travel with individuals to approved activities using public transportation as requested.
