What you'll do

• Review facility policies for regulatory compliance
• participate in resident-care and quality-of-life policies and quality assurance meetings
• document psychosocial needs
• complete social histories and assessments
• coordinate interdisciplinary care plans and discharge planning
• provide or arrange therapeutic interventions, support, education, and referrals
• and educate staff, residents, and families about rights and available services.
