Registered Nurse (RN) - Hospice and Palliative Care Liaison

Facilitate Hospice and Palliative Care referrals, obtain reviews and coordinate information for clinical assessment, determine patient appropriateness for hospice, communicate hospice philosophy and criteria, serve as a consultant to discuss goals of care, develop and maintain knowledge of available services, and complete comprehensive documentation.
