Residential Case Manager

The Residential Case Manager works onsite in Richmond, VA, for $22–$28 per hour. What you'll do

• Assess residents’ physical, emotional, and social needs and develop individualized care plans
• coordinate and monitor healthcare, counseling, and education services
• advocate for resources
• document case activities, progress notes, and service plans
• provide crisis intervention and support
• coordinate communication among residents, families, care teams, and external organizations
• participate in team meetings and case reviews
• ensure compliance with residential-care policies and legal requirements.

Why this role

At least two years of case management or residential care experience required. Social Work or related human-services degrees, veteran or behavioral-health experience, electronic case-management familiarity, and Spanish bilingual ability are preferred.
