Social Services Coordinator

Job overview

The Social Services Coordinator role is fully onsite in Miami, FL, Monday through Friday. Pay is $20 per hour. Solis Health Plans provides Medicare Advantage solutions focused on a personalized experience for members and providers. The coordinator helps members and prospective members navigate applications for Medicaid-related assistance programs, including QMB, SLMB, QI1, and LIS. The role supports access to eligible benefits through outreach, screening, application assistance, follow-up, and coordination with internal teams and government agencies.

What you'll do

• Conduct telephone and mail outreach
• assist members with state and federal Medicaid program applications
• identify and pre-screen prospective members for QMB, SLMB, QI1, and LIS
• interview clients and families and coordinate activities addressing social or emotional needs
• follow up with the Department of Children and Families and Social Security Administration
• answer member questions in person or by phone
• maintain organized case records
• collaborate with Sales, Case Management, and Member Services
• follow applicable CMS, AHCA, HIPAA, and risk-management requirements.
