What you'll do

• Provide in-home patient care
• examine patients, complete assessments, document progress, and report changes to physicians
• educate patients about care plans, medications, therapy, diet, and exercise
• coach family members on care and home safety
• work with physicians and therapists
• oversee LPNs, CNAs, and aides providing care.
