What you'll do

• Conduct patient outreach, check-ins, and assessments through phone, text, video, or in-person encounters
• assist with appointments, office visits, and social-service applications
• coordinate referrals with health plans, providers, and multidisciplinary care teams
• use motivational interviewing, trauma-informed care, and harm-reduction practices
• document services in care-management portals
• manage caseload priorities and follow-up
• conduct community outreach at clinics and partner sites
• and help organize or staff community events. Regular travel across assigned counties is required.
