The Behavioral Health Care Manager I — Level I role is onsite in Chandler, OK. Compensation is not specified. Red Rock Behavioral Health Services’ Health Team coordinates care and supports people with mental, emotional, and substance-use concerns. The Care Manager implements comprehensive care plans addressing members’ mental health, physical health, and other life-domain goals. Through assessment, coordination, advocacy, documentation, and direct clinical services, the role helps connect consumers with appropriate treatment and supports progress toward clinical outcomes aligned with their needs and preferences.

What you'll do

• Implement comprehensive care plans
• provide practical support, advocacy, coordination, problem-solving, direct assistance, training, and supervision to help consumers access daily-living necessities and services
• participate in treatment team meetings when information is pertinent
• assist with developing, implementing, evaluating, and revising service plans
• coordinate approved services and communicate with providers and referral sources
• support consumers’ families
• assist consumers with primary care access and physical-health self-management
• complete service plans, assessments, prior authorizations, and progress notes
• assess symptoms and treatment response and adjust plans as appropriate
• provide individual, group, and family clinical services in office and community settings
• coordinate with inpatient services and link consumers to self-help programs and residential facilities
• support care plans within the Red Rock Health Team.
