Registered Nurse – Home Health

The Registered Nurse – Home Health role is onsite in Albany, KY, serving Clinton, Cumberland, and Monroe counties. Compensation is not specified; mileage reimbursement is 43 cents per mile. The role supports home health care and includes a Monday–Friday, 8 a.m.–5 p.m. schedule with an on-call schedule of one to two weekdays and possibly one weekend per month. The nurse assesses individuals and implements and evaluates care plans to maintain health, prevent disease, and manage care. The role coordinates with providers and other disciplines, monitors changes in client condition, and supports safe medication practices, documentation, appointments, and health-related training.

What you'll do

• Observe, assess, plan, implement, and evaluate nursing support for well, chronic, or acutely ill individuals
• coordinate with health professionals and providers
• communicate changes in client condition to physicians
• ensure medication ordering, review, monitoring, and related policies
• complete nursing documentation and notes
• coordinate health-related appointments and communication with inpatient nursing and discharge coordinators
• train employees and unlicensed staff on health and safety needs
• supervise licensed practical or vocational nurses as needed.
