What you'll do

• Provide Enhanced Care Management-aligned coordination
• conduct patient outreach, engagement, and follow-up
• identify barriers to care
• monitor behavioral health status and escalate concerns appropriately
• facilitate communication across interdisciplinary teams
• maintain timely, accurate documentation per payer requirements
• support continuity across behavioral, medical, and ancillary services.
