Case Manager

The Case Manager position is onsite in Mesa, Arizona, with a stated pay range of $22 to $24 per hour. Save The Family Foundation of Arizona’s Direct Services Programs help families work toward housing and long-term stability. The case manager supports families in securing permanent housing, income, and other resources. The role coordinates service plans, referrals, documentation, and communication with clients and community partners to help families progress toward self-sufficiency.

What you'll do

• Manage a caseload of families
• assist clients with rental housing, paperwork, referrals, and goal setting
• assess needs and barriers
• develop and monitor service plan goals
• coordinate with housing providers and community resources
• maintain client files and program data
• conduct client meetings and home inspections
• coordinate move-ins and move-outs
• prepare reports and attend service team meetings.
