What you'll do

• Provide one-on-one skills training in homes, the office, or community settings
• provide crisis and emergency support through face-to-face or phone contact and on-call availability
• maintain electronic client records and progress notes
• engage clients and families and provide emotional support
• track progress toward service-plan goals
• support daily living skills
• collaborate with families, courts, probation, and community agencies
• participate in meetings and training.
