What you'll do

• Conduct initial and ongoing nursing assessments
• develop and update individualized Plans of Care
• provide skilled nursing and comfort-focused interventions, including pain and symptom management
• coordinate with physicians and interdisciplinary team members
• educate and support patients and families
• coordinate medications, supplies, equipment, and services
• maintain clinical documentation
• participate in interdisciplinary meetings and the required on-call rotation
• follow hospice regulations, practice standards, HIPAA requirements, and agency policies.
