What you'll do

• Provide skilled nursing assessments and interventions
• update care plans and review them with patients and families
• respond to urgent needs with triage staff and initiate crisis care as needed
• report significant changes
• educate patients and families
• administer and titrate medications as ordered
• maintain medication records and electronic medical records
• coordinate with interdisciplinary teams
• delegate nursing duties to LPNs and supervise CNA care
• complete admissions as needed
• conduct telephone triage and follow-up
• coordinate volunteer deliveries.

Why this role

The schedule is 24 hours per week in two 12-hour shifts from 7:00 PM–7:30 AM, includes every other weekend from 7:00 PM–7:00 AM, 1–2 holidays per year, and an on-call rotation of approximately 10–15 shifts per year.
