What you'll do

• Develop referral relationships with hospitals, rehabilitation centers, senior living communities, physicians, social workers, elder law attorneys, and other partners
• prospect for new referral opportunities
• conduct meetings, presentations, networking, and outreach
• follow up on referrals and prospective clients
• help families understand care options
• coordinate case starts with care, clinical, recruiting, scheduling, and operations teams
• maintain CRM and referral records
• track conversions, case starts, census growth, and territory performance.
