What you'll do

• Provide skilled nursing care in participants’ residences
• triage participant and staff calls
• assist with examinations, testing, and treatment
• implement physician orders
• administer medications and treatments
• educate participants and families
• document care and maintain medical records
• assess and reassess needs and changes in condition
• coordinate care plans and referrals
• supervise LPNs and home health aides or CNAs according to stated intervals
• participate in quality and utilization review as assigned.
